Our Services
Individual Supports
Individual Supports are services that provide direct support and assistance for individuals in or out of the individual's residence. Individual Supports encompass everything from providing assistance in basic self-care include to assisting with engagement in community-based activities and developing social skills. Abri Care & Community Supports helps people achieve and maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community.
Abri Care & Community Supports can also work with the family members who are providing individual supports for their loved one in their homes.
Our staff undergo a rigorous screening process prior to employment. We provide a thorough and comprehensive orientation, which includes specialized training in working with individuals with disabilities, intense behavioral support needs, and complex medical care needs. Our staff receive ongoing training and professional development to further enhance their skills and competence.

Community-Based Supports
Community-Based Supports include services that provide direct support and assistance for individuals to achieve and/or maintain independence, productivity, enhanced family functioning, and inclusion in the community. Community-Based Supports include one-one-one training and supervision of individuals as they learn and perform the various tasks that are included in basic self-care and activities of daily living, develop social skills, and participate in community-based activities.
Some examples of Community Based Supports Activities are:

Support from staff to enable an individual to attend an event, take a class, etc.

Support from staff to assist an individual participating in activities such as: assistance in completing activities of daily living, ordering off a menu, purchasing items, learning basic cooking, laundry skills, etiquette, travel training, accessing activities in the community, etc.

One-on-one tutoring

Support on a job site to assist in basic self-care, social skills, and activities of daily living.

Behavioral Supports
Abri Care & Community Supports is developing comprehensive behavioral support services for adults with developmental disabilities. Behavioral Supports offered by Abri Care will include a complete assessment of the challenging behavior(s), development of a structured behavioral modification plan, implementation of the plan, ongoing training and supervision of caregivers and behavioral aides, and periodic reassessment of the plan. Behavioral Supports may include individual and/or group counseling, behavioral interventions, diagnostic evaluations or consultations related to the individual’s developmental disability and necessary for the individual to acquire or maintain appropriate interactions with others.
Behavioral Supports services are offered in addition to and do not replace treatment services for behavioral health conditions that can be accessed through the State Plan/MBHO and mental health service system. Individuals with co-occurring diagnoses of developmental disabilities and mental health conditions shall have identified needs met by each of the appropriate systems without duplication but with coordination to obtain the best outcome for the individual.

Community Inclusion
Abri Care & Community Supports provides community inclusion services that support and assist individuals in educational, enrichment or recreational activities that enhance his/her inclusion in the community. Community Inclusion Services are delivered in a group setting.
Some examples of Community Inclusion Services Activities:

Small group outings to community festivals, museums, book clubs, theater groups, cultural events, holiday celebrations, sporting events, etc.

Natural Supports Training
“Natural Supports” refers to any person, family member, neighbor, friend, companion, or co-worker who provides uncompensated care, training, guidance, companionship or support for an adult with intellectual and developmental disability. Abri Care & Community Supports provides training for Natural Supports on the treatment regimens and other services included in the Service Plan, use of equipment specified in the Service Plan, and updates the training as necessary for the Natural Support to safely provide care at home. Counseling is aimed at assisting Natural Support Natural Support in meeting the needs of the individual.
Abri Care & Community Supports provides training and counseling services for Natural Supports - persons who provide unpaid support, training, companionship or supervision to adults with intellectual and developmental disabilities.

Supported Employment
Abri Care & Community Supports believes individuals with developmental disabilities can fulfill their employment aspirations and achieve social and economic inclusion through employment opportunities. Respite
Care and supports services provided as respite under the New Jersey Division of Development Disabilities Community Care Program and Supports Program includes many options and may be provided in or out of the individual’s home. Generally, respite services provided for individuals on a short-term basis because of the absence or need for relief of those persons who normally provide care for the participant. Respite may be provided in the participant’s home, a licensed community residence (group home), or another community-based setting. Abri Care & Community Supports provides in-home and out-of-home respite care. For more information about your specific needs, please contact us.
