Community-Based Supports

 

Community-Based Supports include services that provide direct support and assistance for individuals to achieve and/or maintain independence, productivity, enhanced family functioning, and inclusion in the community. Community-Based Supports include one-one-one training and supervision of individuals as they learn and perform the various tasks that are included in basic self-care and activities of daily living, develop social skills, and participate in community-based activities.

Some examples of Community Based Supports Activities are:

  • Support from staff to enable an individual to attend an event, take a class, etc.   

  • Support from staff to assist an individual participating in activities such as:

    •  assistance in completing activities of daily living, ordering off a menu, purchasing items, learning basic cooking, laundry skills, etiquette, travel training, accessing activities in the community, etc.

  • One-on-one tutoring

  • Support on a job site to assist in basic self-care, social skills, and activities of daily  living.

Our staff undergo a rigorous screening process prior to employment. We provide a thorough and comprehensive orientation, which includes specialized training in working with individuals with disabilities, intense behavioral support needs, and complex medical care needs.  Our staff receive ongoing training and professional development to further enhance their skills and competence.
